Dr. Kelly Fuqua, MD – Family Medicine
Board-Certified in Family Medicine
Dr. Kelly Fuqua has been caring for families in South Louisiana since 2009, serving the Sulphur and Carlyss communities for over 15 years. A graduate of LSU School of Medicine in New Orleans, she brings extensive knowledge, clinical experience and a compassionate, patient-first approach to family medicine.
Known for her thoroughness and ability to make patients feel heard, Dr. Fuqua is especially passionate about preventive care and helping patients manage chronic conditions with confidence.

Dr. Jason Fuqua, MD – Family Practice
Board-Certified in Family Practice
Dr. Jason Fuqua has been caring for patients in Carlyss and Sulphur while also serving on the medical staff of West Calcasieu Cameron Hospital for the past 16 years. A graduate of McNeese State University and LSU School of Medicine in New Orleans, he completed both his internship and residency at East Jefferson General Hospital.
With more than 15 years of experience in family practice, Dr. Fuqua is known for his calm, patient-centered bedside manner and his dedication to listening and understanding those he cares for.

Garett Istre, APRN, FNP-C – Family Nurse Practitioner
Board-Certified Nurse Practitioner
Family Nurse Practitioner Garett Istre brings more than a decade of nursing experience to the Carlyss community. A McNeese State University graduate with both bachelor’s and master’s degrees in nursing, he is certified by the American Academy of Nurse Practitioners and has served patients across Southwest Louisiana in family practice, public health and clinical settings.
Garett is known for his thorough approach, professionalism and ability to make patients feel at ease while ensuring they receive comprehensive, high-quality care.
